A plane crash, killing almost an entire Brazilian Football team, has been explained. The plane operators violated some standards. They neglected to have a refueling stop, and the plane plain ran out of fuel There's been some finger pointing about that. An airport official said she warned the plane crew that they needed to fuel up before leaving, but the crew assured her they had enough.
